PROGRAM GUIDELINES
Eligibility
The Health Foundation, Inc. supports programs and services that improve the health and well-being of the citizens of our area. Support is generally limited to: 1) non-profit organizations with tax exempt status as described in Section 501(c)(3) of the Internal Revenue Code or corporations with tax exempt status as described under Section 170 (c)(2) of the Internal Revenue Code; or 2) scholarships to individuals pursuing an education in health related fields who are committed to locating in our area.
Non-Eligible Programs
The Health Foundation’s mission does not include financial support of the following:
- Religious organizations, churches, synagogues or other organizations that provide a service which is primarily religious in nature.
- School districts, individual primary or secondary schools (public or private).
- Organizations and activities associated with elementary and secondary schools (either public or private) such as PTA’s, PTO’s, booster clubs, carnivals, etc.
- Civic clubs, social or fraternal organizations.
- Individuals (except for scholarships).
- Annual operating funds.
- Athletic programs, events, facilities and organizing committees.
- Tables, galas, tickets to "benefits" and other fund raisers for annual operating funds or special projects.
- Political organizations and activities.
- Memorials.
APPLICATION GUIDELINES
Non-profit organizations seeking funding should be able to provide the following information to The Health Foundation. The Executive Director will aid the applicant in determining what items from the list below are required. Applicants may be asked to submit their request orally or in writing, depending on the nature of the request:
- A description of the organization, its history, purpose and programs.
- The amount of the contribution solicited and a budget for the project including the campaign goal.
- A concise description of the project including the specific purposes for which the requested contribution is to be used.The amount raised to date, including a list of corporate gifts, pledges and plans for procuring the remainder.
- A list of the organization’s board of directors and key staff.
- A current annual report or financial statement.
- Copy of IRS forms certifying that the organization is exempt from federal income tax payment under Section 501(c) (3), including a tax-exempt charter exemption letter.
Individuals seeking funds under the Foundation’s scholarship program shall:
Complete a scholarship application provided by the Foundation.Comply with such other criteria as established by the scholarship committee.
FUNDING APPROVAL GUIDELINES
The Executive Director works with applicants to determine whether a proposal meets the eligibility guidelines as established by the full board.
Either a written proposal or a synopsis of requests meeting the criteria are submitted to the ProgramCommittee for review.
The Program Committee will meet on an as-needed basis to review applications.
